چکیده

Morphologic changes in the coronary arteries of patients with ischemic heart disease were evaluated by angiography. Although a powerful diagnostic tool, angiography can show only the shadow of the luminal changes and therefore is an insufficient tool for diagnosis of pathological changes. Recent advances in fi beroptic technology now enable observation of the luminal changes even in small diameter vessels. The fiberscope is now used for observation of experimental thrombosis and thrombolysis; laser irradiation induced changes in cadaveric hearts and vessels; luminal changes in peripheral vessels and the pulmonary artery, laserinduced changes in femoral arteries in patients with obstructive atherosclerosis; thromboemboli in patients with pulmonary embolism6); cardiac chambers and valves in animals7) and in various categories of heart disease in man,8)-10) and for guidance of endomyocardial biopsy.11),12) The angioscope is also used for observation of the coronary arteries during coronary artery bypass surgery. Recently, the coronary artery has also been observed during cardiac catheterization.15)-18) In this study, we describe percutaneous angioscopy of the coronary artery during routine cardiac catheterization or intervention.

Percutaneous Transluminal Angioscopy During Coronary Intervention

To investigate the feasibility of angioscopic-guided percutaneous transluminal coronary angioplasty and to elucidate the mechanism of efficacy of coronary stenting for acute myocardial infarction, we performed coronary angioscopy in 102 patients with stable angina or acute myocardial infarction. Thrombi and intimal flaps were observed in most patients after coronary angioplasty. Large intimal s...

Serial angioscopic and angiographic observations during the first hour after successful coronary angioplasty: a preamble to a multicenter trial addressing angioscopic markers for restenosis.

Percutaneous coronary angioscopy was used in 13 patients in a pilot study to assess the intracoronary changes that occur during the first hour after balloon angioplasty (PTCA). The dilated segment was studied with 4.5F angioscopes and with quantitative coronary angiography (QCA) immediately after PTCA and at 15-minute intervals for up to 1 hour after PTCA. Significant progression of intimal dis...

Advances in Angioscopic Imaging of Vascular Disease

Percutaneous angioscopy, using high resolution fiberoptic imaging, allows direct and two-dimensional visualization of the vascular interior, thereby enabling macroscopic pathological diagnosis. Percutaneous angioscopy has revealed that the vascular luminal surface exhibits various colors and morphologies characteristic of different vascular diseases. This imaging technique is used for evaluatio...

Detection of coronary plaque by computed tomography with a novel plaque analysis system, 'Plaque Map', and comparison with intravascular ultrasound and angioscopy.

BACKGROUND Previous reports suggest that plaque may be characterized by the computed tomography (CT) number, but there is not a comprehensive method for evaluating the gray-scale CT image of the coronary artery obtained by multi-detector row CT (MDCT). METHODS AND RESULTS Forty-five patients with acute coronary syndrome (ACS) underwent MDCT either 3-4 weeks after the onset of acute myocardial...
